Appointments
Please call the office, in advance, to make an appointment. If
you're unable to keep your appointment, please call us as soon
as possible so we may use that time to see another patient in need
of care. We make a sincere effort to adhere to appointment times and appreciate your patience if we are
running behind schedule due to emergencies or
hospital surgery.
Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals
between 8:30 a.m. - 4:30 p.m., Monday through Friday, when full patient
records are available.
